San Paolo Maggiore in Naples stands on the site of a first-century temple of the Dioscuri, with the church built behind its front section. The Theatine order received it in 1538, and a general reconstruction began in the early 1580s. Francesco Grimaldi designed the adjoining house, Giovan Giacomo di Conforto built the aisles from 1625, and Ferdinando Fuga made the main altar in 1775 and 1776. The text gives no completion year.

The style is Baroque on a Latin cross plan. Marble from the ancient building was reused for the pavement and nave pilasters, and two Corinthian columns from the temple stand before the facade. The text blames the 1688 facade collapse on both an earthquake and a wall that Dionisio Lazzaro built.

Allied bombing in 1943 severely damaged the church and most of Massimo Stanzione's nave frescoes. St Cajetan is buried here. It is a minor basilica. The article gives no hours.
