Pope Innocent III ordered the cathedral moved from Pionta Hill into the city walls in 1203. The current structure was begun in 1277 and completed in 1511, and it is predominantly Gothic. The south side is sandstone from the medieval building, and the south portal has two porphyry columns reused from an ancient temple. The article gives no dimensions.

Guillaume de Marcillat made the stained glass from 1516 to 1524, and Giorgio Vasari made the wooden choir in 1554. Piero della Francesca's Mary Magdalene dates to the 1460s. The Tarlati tomb is attributed by some to Giotto, which the article does not confirm.

Dante Viviani designed the facade, built from 1901 to 1914 to replace an unfinished 15th-century one. It is an active cathedral and seat of the Bishop of Arezzo-Cortona-Sansepolcro.
